Most ankle sprains affect the ligaments on the outer side of the ankle, particularly the anterior talofibular ligament (ATFL) and calcaneofibular ligament (CFL). Depending on the extent of ligament damage, ankle sprains are generally classified into three grades.
A Grade 1 sprain is the mildest type of ankle sprain and involves stretching of the ligaments without significant tearing. Symptoms typically include mild pain, tenderness and slight swelling, with little or no ankle instability. Most people are able to walk with minimal discomfort, and recovery usually takes one to three weeks with appropriate treatment.
A Grade 2 sprain involves a partial tear of one or more ankle ligaments. This often causes more noticeable pain, swelling and bruising, along with some loss of ankle function and difficulty bearing weight. With appropriate treatment and rehabilitation, recovery typically takes three to six weeks.
A Grade 3 sprain is the most severe form of ankle sprain and involves a complete tear of one or more ligaments. Symptoms may include significant swelling, bruising, marked ankle instability and considerable difficulty walking or bearing weight. Recovery can take eight to 12 weeks or longer, depending on the severity of the injury and whether surgical treatment is required.
Treatment depends on the severity of the injury, the degree of ligament damage and whether ankle instability is present. Most ankle sprains can be managed without surgery, although surgical intervention may be considered in selected cases.
The RICE method is often recommended during the early stages of recovery.
Many ankle sprains respond well to conservative management.
Surgery may be considered for severe ligament injuries, chronic ankle instability or recurrent ankle sprains that have not improved with non-surgical treatment.
